New Patient

In order to make an appointment at the office, you will need a referral from another doctor.  The referring doctor and our office will work together in making you an appointment.  You will be notified after your appointment is made with our office.  Once you see one of our doctors you must remain with that doctor.

Our practice participates in the following insurance:

  • Medicare
  • Blue Cross (Preferred Care & PPO)
  • Aetna
  • Health Choice
  • Health Network
  • United Health Care
  • Secure Horizons
  • VIVA

If your coverage is with one of these companies we will file your claims for you.  If your insurance is not listed you are expected to pay any outstanding balance along with your charges that have occurred for that day’s visit.  You are expected to pay your copay at the time of departure.  If you do not pay your copay, we will add an additional charge of $10.00.

If you have an HMO which requires a referral to be seen by us, remember that is the responsibility of you and your primary care physician.  This is the policy of your insurance copmany.  We are not allowed to obtain referral for any of your visits at our office.

If you have United Health Care or Medicare Complete, contact your primary care physician before every visit to make them aware that you are being seen by one of our physicians.

Medicare deductibles are due at the beginning of January.  You are responsible for this deductible unless you have a secondary insurance that will pick up the deductible.  If copies of your records or lab reports need to be sent to another doctor, please inform your doctor at the time of your visit.  If you, or an insurance company, or an attorney’s office needs your records, we must first have a signed authorization of release to send the records.  There are fees for completion of forms and copies of records released to the patient. 

Our house calls system will contact you 48 hours before your appointment to remind you of your appointment date and time.  If you have a conflict in your scheduled appointment, please notify our office as soon as you are aware and we will do the same.  No patient will be seen without an appointment. 

When you call our office, you will get an automated attendant.  If you need to schedule, reschedule, or cancel an appointment, press 2.

As a new patient, you are expected to arrive 15 minutes prior to your appointment.  You will need to bring your photo id, insurance card, and new patient paperwork with you.  Please be aware that your initial appointment may take up to 2-3 hours, as you may have X-rays and/or blood work at this time.
